THE 5 STAGES OF TIME EFFICIENT TRIAGE
Last update 12/25/2024 - 16:07
In an emergency situation the biggest threat a patient can face is often time. To ensure stabilization, treatment and recovery, the A&E team receiving the patient must make multiple controlled and measured medical decisions, in rapid succession.
When a patient first presents at A&E the first of these decisions is key – assessment, known in medicine as “Triage”. As every patient must be triaged immediately, an A&E team must show experience, knowledge and composure from the moment a patient arrives.
At Hanoi French Hospital, the A&E team makes sure their response time and decision-making are of utmost importance, as they follow a time-efficient International Triage System. This allows fast treatment and clear communication.
While assessing a patient, there is also one more important factor the HFH team take into consideration.
HFH’s A&E Director Dr. Francois Pruvot, who has over 20 years’ experience across Emergency Medicine, General Medicine and Disaster Medicine, adds: “At HFH we take your pain into account, and always try our best to make your journey at hospital as pleasant as possible. Before doing any procedure in AE we want you to feel safe and in less pain. For this, we have different techniques and medication, from the right immobilization to IV pain killer and nerve blocks with our anesthesiology team.”
Every patient at Hanoi French Hospital’s A&E Service is assessed by our Emergency Medicine team of medical professionals considering, but not limited to: medical past and present, physical examination of current condition and additional specialists’ consultation if needed. As time is of the essence in these situations, the patient will be assigned a crucial and decisive time-frame for treatment. There are five key stages a patient can fit into:
Immediate
Any patient suffering from cardiopulmonary arrest, shock of any etiology (cardiac, septic, hemorrhagic etc.), respiratory failure with need for ventilation, epileptic state, anaphylaxis, severe trauma, any condition with impaired consciousness, airway symptoms after smoke inhalation or burn injury, or severe hypoglycemia.
If a patient is triaged with any of these symptoms or ailments they will be classified as needing immediate treatment. Emergency interventions, resuscitations and procedures are performed on all critical patients within the most-immediate window of opportunity by our internationally trained Emergency Doctors, Surgeons and medical professionals.
< 10 minutes (Very Urgent)
Any patient suffering from myocardial infarction (STEMI ‘heart attack’, NSTEMI ‘heart attack’ with persistent pain), acute respiratory problems, hemoptysis (coughing up blood from internal airway bleeding), hematemesis (vomiting blood), acute stroke within time frame for fibrinolytic therapy, intracranial hemorrhage, postictal state after epileptic seizure, high fever with chills, meningitis, severe sepsis, impaired consciousness, acute psychosis, substance intoxication, hypoglycemia, ketoacidosis (complication of diabetes mellitus).
Patients requiring treatment urgently will be assessed and managed within 10 minutes of presentation at A&E in Hanoi French Hospital by our Emergency Doctors.
< 30 minutes
Any patient suffering with unstable angina, myocardial infarction without high severity (NSTEMI ‘heart attack’ without pain), abdominal pain, stable hemorrhage, continuous vomiting, pulmonary embolism (stable), hypertensive crisis, presentation after syncope (fainting), heart failure (stable), febrile infection (fever of above __ due to infection)
The complexity of each individual patient’s current condition will determine the proper response time and treatment will be tailored and monitored according to their condition in every emergency situation, including those requiring treatment within 30 minutes of presentation.
< 90 minutes
No real or urgent emergency situation (i.e. a common cold) etc. but patient will be assessed, treated and referred to the correct in-house or other healthcare facility based on their condition.
< 120 minutes
